To help the medical center and PHO decide whether to develop an ACO and if so, how to define its network and develop the capabilities required to succeed.

Process

Veralon completed a preliminary assessment that provided a high-level estimate of the market size of the opportunity, and an initial review of the PHO’s capabilities and gaps as they relate to ACO requirements, and potential next steps.

Results

The client was convinced of the value of developing an ACO. Veralon then developed a preliminary work plan for the PHO, highlighting the elements and timeline required for ACO planning and development.

The PHO is now moving forward with a Pioneer ACO application, which provides a faster path for ACOs that have already begun coordinating care for patients.
